United Laboratory Network IPA, LLC (hereinafter "ULN") arranges for the provision of clinical laboratory services to members of health maintenance organizations. ULN contracts with clinical laboratories who agree to become Participating Providers to provide services for members of health maintenance organizations with which ULN has a contract. ULN may receive patient personal and individually identifiable health care information either from a contracted health maintenance organization or a contracted clinical laboratory provider.
ULN is committed to protecting the privacy of all patient personal and individually identifiable health care information that ULN may receive. Privacy standards apply to all types of personal and individually identifiable health care information collected whether in electronic or hard copy format and whether original or copied.
ULN uses safeguards and techniques designed to secure personal and individually identifiable health care information. It limits access to information to those who specifically need it to conduct their business responsibilities and only permits authorized employees, contracted business associates that agree to privacy standards, clinical laboratories participating in ULN's Participating Provider Network, and contracted health maintenance organizations to have access to that information.
ULN does not share personal or individually identifiable health care information for any purpose other than for the servicing of a claim or account, administration of a health maintenance organization contract, or other purposes permitted by law without prior written authorization of the HMO member, which may be obtained by the HMO or a clinical laboratory. Servicing of a claim or accounts would include, as an example, determining the amount to be paid to a clinical laboratory for services provided. Administration of a health maintenance organization contract would include, as an example, participating in quality assurance activities. Aggregated data that does not disclose personal or personally identifiable health care information also may be created and released. ULN does not sell any personal or individually identifiable health care information.
Changes in Information
For an individual to make changes in personal or personally identifiable health care information, the individual should contact either the health maintenance organization or the clinical laboratory that is the source of the information.
Changes in Policy
ULN may make changes to this policy without prior notice. In the event this occurs, the revised policy will be posted in a timely manner on the ULN website.
